Agrace Hospice & Palliative Care (Agrace), UnityPoint at Home, which operates locally with UnityPoint Health – Meriter, and UW Health today announced a new affiliation agreement, with the goal of partnering to serve the end-of-life needs of people in southern Wisconsin.
